چکیده

The Holt-Oram syndrome is an autosomal dominant heritable disorder characterized by skeletal upper-limb dysplasias and congenital cardiac defects. We describe a 43-year-old woman who presented with paroxysmal tachycardia and progressive heart failure. Both ring fingers were abnormally short as a result of dysplasia of metacarpal IV (Figure 1). Auscultation revealed a loud systolic murmur at the left sternal margin and a widely split S2. The ECG was consistent with right ventricular hypertrophy (Figure 2). The chest x-ray showed severe right atrial and right ventricular dilatation and marked bilateral central pulmonary arterial distension with small peripheral pulmonary vessels (Figure 3, left). Echocardiography showed a large secundum-type atrial septal defect with signs of severe right ventricular volume overload. At right heart catheterization, oximetry showed a large left-to-right shunt at the atrial level with a Qp/Qs ratio of 2.3. The mean pulmonary arterial pressure was 53 mm Hg, and the wedge pressure was 13 mm Hg. A pulmonary arteriolar resistance of 851 dyn z s z cm was calculated. Operative closure of the atrial septal defect was undertaken despite markedly elevated pulmonary arteriolar resistance. Three months after the procedure, the patient reported excellent clinical improvement: there was no murmur, the size of the heart and the radiological signs of pulmonary hyperperfusion had decreased significantly (Figure 3, right), pulmonary artery mean pressure was 14 mm Hg, and pulmonary arteriolar resistance had decreased to 412 dyn z s z cm.

A Case of Holt - Oram Syndrome

SUMMARY Holt Oram Syndrome consisting of upper limb defcts, cardiac anomalies and narrow shoulder, was first described by Holt and Oram in 1960. The inheritance pattern is autosomal dominant and most commonly encoutered cardiac anomalies being ASD, although all variaties of C.H.D are reported. Upper limb anomalies are not specific, although scaphoid bone deformity is almost characteristic in t...

A genetic Assay of Three Patients in the Same Family with Holt-Oram Syndrome; a Case Report

Holt-Oram syndrome (HOS) is a developmental disorder inherited in an autosomal-dominant pattern. Affected organs are the heart and forelimbs with upper extremity skeletal defects and congenital heart malformation. In this study we present three cases of HOS in the same family. In one of these three individuals we detected a transition of C to T (CTG-GTT, V205V) in exon 7 of the TBX5 gene. This ...

Holt-Oram Syndrome: A Rare Variant

Holt-Oram syndrome is an autosomal dominant disorder, characterised by skeletal abnormalities of the upper limb associated with congenital heart defect, mainly atrial and ventricular septal defects. Skeletal defects exclusively affect the upper limbs in the preaxial radial ray distribution and are bilateral and asymmetrical. They range from clinodactyly, absent or digitalised thumb, hypoplastic...

Perinatal/neonatal case presentation: unexpected severe respiratory insufficiency in a newborn with Holt-Oram Syndrome.

Holt-Oram syndrome is an autosomal dominant condition characterized by skeletal and cardiac defects. Pulmonary malformation is not reported to belong to the spectrum of this condition. We report a second case of a newborn with Holt-Oram syndrome who developed severe respiratory insufficiency shortly after birth. We discuss possible genetic links between abnormal pulmonary morphogenesis and Holt...

Atrial Fibrillation and Other Clinical Manifestations of Altered TBX5 Dosage in Typical Holt-Oram Syndrome.

Atrial Fibrillation and Other Clinical Manifestations of Altered TBX5 Dosage in Typical Holt–Oram Syndrome To the editor: We were pleased to read the recent study in Circulation Research by Postma et al1 that describes an activation mutation in TBX5 that causes Holt–Oram syndrome. These exciting findings validate prior studies (reviewed elsewhere2) showing that cytogenetic abnormalities that pr...

Prenatal sonographic diagnosis of familial Holt-Oram syndrome associated with type B interrupted aortic arch.

We present a rare case of familial Holt-Oram syndrome diagnosed sonographically at 18 weeks of gestation. The foetus had serious bilateral upper limb malformations, a ventricular septal defect and a type B interrupted aortic arch, while the mother had bilateral upper limb malformations only. The pregnancy was terminated. A pathological and radiological examination of the foetus confirmed the pr...
